In addition to the application, persons with a medical or pyschiatric history are required to fill out a "Release of Clinical Record Information " form upon which a "tenant informaton sheet" that includes the clinical information should be submitted. The information sheet is used for the purpose of emergencies only and is kept purely confidential. These forms may be printed by clicking on the words highlighted. Over and above these the applicant should read and sign the "rules for all tenants" that he agrees to abide by them.

Prerequisites for Living at the Champion's Centre

The criteria for anyone living at the Champion's Centre is simply low-income. However, because our housing structures to this point have been renovated buildings, they are not conducive to people with a physical disability.

Anyone wishing to live at the Champion's Centre should be comfortable using common areas with at least another 11 people and walking up a flight of stairs (depending upon location) . In addition, the applicant should be comfortable with the fact that the Champion's Centre Inc. is a Christian organization. Hence, each Centre will have Chapel Services (volunteer attendance) on Sundays open to the tenants and public alike, and possibly mid-week Care Groups (optional).

Each individual will pay a one-time refundable security deposit of one month's rent (key deposit of $30 included) and a reasonable rent each month, which includes one hot meal a day, (optional) room cleaning and linen changing (optional) once a week and all utilities. A continental breakfast and lunch consisting of bread, spreads, muffins, oranges, apples etc. are provided but tenants are required to prepare their own.

Individuals in shared rooms (2 people) pay a security deposit of one month's rent per person, the rent being about 20% less than a single room occupancy.
